The following from is to be completed and signed by parents or guardians if they will not be present at a ride with a junior rider. If parents or guardians will be at the ride, please sign the Junior Permission and Waiver form that is on page 2 of the ride entry form.

This is to be completed for each and every ride.  This conflicts with rule 27.7 clause c) in our national rule book which says it is not required if the rider is a full riding junior member and the Junior Permission and Waiver was signed when the membership application was completed.  Our insurance company has advised that this is not acceptable and a Junior Permission and Waiver form must be complete at each ride.
